Getting Started
Many patients wait for a long time for an appointment so we try our hardest to make our office run efficiently.
We charge a $75 fee for missed new patient appointments not canceled 24 hours prior.
We are in-network with the majority of insurance plans. However, we do not accept BCBS HMOs. All Medicaid plans need a referral from your primary care physician (PCP).
We will make every effort to review your insurance information prior to your visit and provide you with the most accurate out-of-pocket cost estimate possible from your insurance company. Since it is the patient’s responsibility to verify his or her insurance coverage, we recommend that patients check the following:
- Are we in-network with your insurance?
- Which services are covered under your plan?
- What are your copayment, deductible and/or coinsurance responsibilities?
Patients without insurance or a high deductible are encouraged to speak to our billing team to discuss financial options. Please call our office for more information.

Patient Portal
You must register for the Patient Portal and stop antihistamines and some other medications before your first visit. See below. The patient portal will allow you to complete your new patient forms prior to your appointment. Please come in 20 minutes early if you are unable to complete the forms on the portal.
In order to have allergy testing performed, you will need to stop your antihistamines 5 days before your appointment. If you are unsure if you should or can stop a medication, please contact our office at leannebaxley@aadtc.us or call 850-386-6680.
Medications to Stop
In addition to registering at the Patient Portal, you must stop taking antihistamines 5 days prior and other medications, such as mood stabilizers 5 days prior. A complete list is available at Medications to Stop.
